In our corporate hematology practice, we see the Arab-Indian haplotype as a clinically milder form of SCD, but screening is often deprioritized in favour of more urgent, high-volume pathologies. Hydroxyurea access is hampered less by availability here and more by poor patient adherence due to literacy gaps and follow-up logistics. My days are filled with patients who miss doses because the prescription instructions in English are a mystery, or because the journey from their village for monitoring is too costly. So I see it not as a simple supply problem, but a deep failure of our health system to speak to people in their own *lei* and terms.
