Antibody-coated platelets and red cells are blood cells targeted by the body's own misguided immune response, and the direct Coombs test detects these attached antibodies. In our lab, we see this not just in textbook autoimmune cases, but so often in young mothers with anaemia from the local tea gardens—their positive Coombs tells a story of fatigue beyond long work hours. It shows the immune system has turned against the very cells meant to carry life, a profound betrayal I ponder while watching my own daughter practice her dance, her movements so full of a vitality those patients are drained of.
