Trastuzumab deruxtecan works in HER2-low because even that faint signal is enough for delivery, but the potent drug can sometimes spill into lung tissue, causing inflammation. In our Delhi trial, we saw it happen in the woman who reminded me of my dadi—same stubborn hope, same fragile frame. That is why I watch the CT scans like my dadi listens for the mustard seeds—knowing the exact moment the hiss turns to a burn.
