Neoadjuvant is the treatment they give you *before* surgery to shrink the tumour, and adjuvant is the *after*, to mop up any stray cells. A pathological complete response is the big hope—it means after that pre-surgery treatment, when they operate, they find no cancer cells left in the tissue. For an old man like me with a weak heart, the neoadjuvant feels like the harder climb because you face the chemo’s fury while still carrying the tumour; my son Tony talks of this ‘pCR’ like a exam result, but I’ve seen my neighbour not survive the intensity to even reach the surgery. So, the difference for me isn’t in the timing, it’s in which battle your body can endure first.
