As a doctor, I understand the textbook answer about bradykinin buildup with ACE inhibitors, but in my primary health centre, I see that dry cough often comes from cheaper generic ACE inhibitors we must stock due to budget. I have switched many patients to ARBs when possible, even though they cost more, because that stubborn cough makes them stop medication entirely. So, for my people, the difference is often less about the mechanism and more about which drug we can afford that they will actually take.
