In the city, they discuss these drugs by hormone receptor status, but here at our centre, tamoxifen is what we have—it works for pre- and post-menopausal women, and we can start it with just a history and a clinical exam. The aromatase inhibitors, they are for post-menopausal only and need more tests we often cannot do. My own sngewbha watching the clouds settle reminds me: sometimes you treat with what reaches the patient, not just what is perfect in the books.
