Frailty scoring is not just a medical checklist; it is a way to see the whole person. Fried’s five criteria—unintentional weight loss, exhaustion, low activity, slow gait, and weak grip—are a clinical framework. But in my clinic, I more often use the Clinical Frailty Scale from 1 (very fit) to 9 (terminally ill), as it captures function and cognition better for complex urban elders. This comes from seeing the scale fail for my own father in Bhubaneswar. His grip was strong, but his loneliness after my mother’s passing made him a 6 on the CFS, something Fried’s list would have missed entirely.
