The patient must sign it themselves if they are an adult and of sound mind. The form should clearly state the surgery name, risks, and benefits in a language they understand. I know because last month, before my knee operation, I sat alone in that cold room and signed my own form, reading each line slowly. They say a family member can, but when your children are far away in Dubai and Hyderabad, you learn to be your own witness.
