Oral cancer screening by visual inspection is a low-cost method where trained health workers check the mouth for early signs. The Kerala and Mumbai trials found it could reduce deaths in high-risk groups like tobacco users. But in Sikkim, our hills have too few trained workers and people come only when pain is severe, not for a simple check. So I put our limited funds into school de-addiction programmes instead, because I see every evening how the children listen—and I hope their fathers will too.
