Ah, Wilms tumour is a kidney cancer of children. In my books, it says cooperative trials across hospitals turned it from often fatal to nearly ninety percent cured. But here in the hills, I see something else—the trials assumed every child reached a big hospital in time, which is not our truth. So my next trial design must first ask: how do we build the road to the hospital, not just the medicine inside it? This comes from watching our own NGO ambulance struggle on these muddy roads, carrying more than just a patient.
