In theory, they are a marvel—designing a vaccine from a patient's own tumour mutations to train the immune system. In our practice, I have seen them work, but only for a few, and the cost is staggering, like building a private highway for one car. What stays with me is the father who sold his ancestral land in Haveri for this hope, only for the melanoma to progress in six months. We celebrate the science, but from my desk, I see more broken finances than lasting remissions.
